Below you will find our templates for the October 31st deadline, wherein we do everything for you except fill in the answers.   Copy and use verbatim each template provided.

Prepare and send an email addressed to us - mpwcfoundation@gmail.com, between October 1st and October 31st of every year.   The subject of this email should be "EXAMP-2012- Section A Confirmation".   You should copy and paste the below templates (emails and/or attachments) (nothing less, nothing more) and send them to us after inserting your own answers.  Be sure to conform to the following color-coding.

Things you should read, then possibly change to ENTER your own information, and then if changed, change the color coding to BLACK

Things you should read as they are instructive in nature, but you should delete or erase them (and close up the space) before sending them to us. (GREEN)

Things that ask questions or make statements that will remain on the input, but which will be followed by the below... (NAVY)

Responses to the above questions or statements from us, that respond to the above questions or statements. (BLACK)

Changes (this year's editings, deletions, additions) to the above that was otherwise THE SAME as you sent us last year.  Note that in these cases, after the grants have been paid, the changes are to be re-colored to match and become the ongoing response. (RED).

It is our desire to continue in the MPWCF's grant program for the grant year 2012.
We have reviewed the 20 questions on http://www.mpwinsma.com/require-1.htm and submit the attached updated answers (using the above color-coding to differentiate this year's changes, if any, from last year's answers).  The attachment is called "EXAMP-2012- 20 Questions"    See below for the format to the 20 questions attachment.
We have re-read some additional details on your web-site page http://www.mpwinsma.com/some_details.htm and will follow its instructions and use THAT method for how to REPLY to emails (especially the long and graphically detailed section named "Regarding EMAILs and REPLYs to emails" near the bottom of the page").   
We have already been working and currently continue to work on our responses to your last year's  "Comments .. due by Jan 15" email and are considering incorporating each such comment in our  "Due by Jan 31st input"

The following response to our 20 questions would probably be more EFFICIENTLY prepared if you had an ONGOING AMENDABLE LIST of the answers to these questions and you submitted it WITH MARKINGS (to differentiate) the current year's changes, if any, on this list.    Then use that list (again next year, as once again amended and updated) for future years.

1-Have you read and completely understood our entire Eligibility page and now send us this written statement affirming that you have met (and still meet) every requirement under the heading "Each new grantee organization MUST:"

2-Do you state what methods you use to ensure continuity and consistency of your programs through re-invigoration and eventual succession of your current management?  Your response may refer to an updated version of your  annual attachment which will be due Jan 31st that states in much greater detail just what you are CURRENTLY doing to assure continuity

Yes, we have already started writing the responses and we will include them  in a detailed statement called "EXAMP-2012-Continuity" using your continuity web-page as a starting point.  This statement will use the template on the due Jan 31st webpage as a starting point to aid our answering this.

3-Please state the number of years you are in our grant program

After we received your grant in 2011, it was the completion of our year #8

4-Write a complete description of your purpose, mission, and/or goals (from your bylaws or conception documents)

We will illustrate this in our comparative and transparent financial statements which we will send to you before Jan 31st.

15-Describe how you project future cash flow (both in and out), (once again, see examples)

Although our budget for next year will be included in our comparative and transparent financial statements which we will send to you before Jan 31st, we now estimate our cash flow (detailed by year and by category) for the next 5 or more years in an attached spreadsheet named "EXAMP-2012-Cashflow Projections"  

16-Describe your past methods of (and future plans for) obtaining funding from sources other than us.
